Position Overview:
The Senior MSW effectively communicates and collaborates with all members of the healthcare team to ensure patients receive safe, timely, and comprehensive services. You will interpret and communicate complex patient and family needs, serve as a role model for caring practices, and assess psychosocial risks based on prior functioning, support systems, and reactions to illness.
Your responsibilities include conducting mental health assessments, facilitating complex discharge planning--including guardianship, foster care, and mental health placements--and providing crisis intervention. You will utilize de-escalation techniques to support high-risk situations and promote safety.

Key Responsibilities:
  • Collaborate with healthcare teams to deliver safe, effective patient care and discharge planning.
  • Assess patients' and families' psychosocial needs, including prior functioning, support systems, and reactions to illness.
  • Counsel patients and families regarding emotional, social, and financial impacts of illness/disability.
  • Access and mobilize community and family resources to meet patient needs.
  • Conduct mental health assessments upon physician referral and provide therapeutic interventions.
  • Support complex discharge planning including guardianship, foster care, adoption, and mental health placements.
  • Serve as the primary point person for cases involving child abuse/neglect, adult/elder abuse, institutional abuse, and domestic violence.
  • Liaise with protective services, law enforcement, schools, probation, and other agencies.
  • Utilize crisis de-escalation, interviewing, and clinical intervention techniques to support high-risk and dangerous situations.
  • Lead clinical interventions during emergencies to ensure patient safety and stability.
  • Develop and maintain strong working relationships with community agencies and healthcare teams.

Qualifications & Requirements:
  • Education: Master's Degree in Social Work required.
  • Licensure: Permanent LSW licensure required; LCSW licensure preferred.
  • Certifications: De-escalation certification required within 6 months of hire.
  • Experience: 3-5 years preferred in mental health, child and family services, adult protective services, Veteran's Administration, or substance abuse.
  • Additional Skills & Knowledge:
    • Knowledge of disease impact and human behavior for assessments and crisis intervention.
    • Understanding of legal mandates related to protective services.
    • Principles of growth and development across the lifespan.
    • Proficiency in crisis de-escalation and interviewing techniques.
    • Ability to develop strong relationships with community agencies, law enforcement, and healthcare providers.
    • Proficiency in Microsoft Excel and Word.
