Position Overview:
The Social Worker effectively communicates and collaborates with all members of the healthcare team to facilitate patient care, ensuring safety, timely service, and comprehensive education for patients and families. This role involves assessing psychosocial risk factors, providing therapeutic interventions, and serving as a liaison with protective services and community resources. The Social Worker also supports complex discharge planning and addresses issues related to abuse, neglect, guardianship, foster care, and mental health placements.

Key Responsibilities:
  • Communicate and collaborate with healthcare team members to ensure safe, timely patient services.
  • Interpret and communicate complex patient and family needs, modeling caring practices.
  • Assess psychosocial factors through evaluation of prior functioning, support systems, reactions to illness, and coping skills.
  • Counsel patients and families on emotional, social, and financial impacts of illness/disability.
  • Access and mobilize community and family resources to meet identified needs.
  • Conduct mental health assessments upon physician referral and provide therapeutic interventions.
  • Facilitate complex discharge planning to ensure smooth transitions.
  • Serve as the point person for cases involving child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
  • Act as liaison between IU Health and protective agencies, foster care, guardianship, and mental health providers.

Qualifications & Requirements:
  • Education:
    • Master's Degree in Social Work (MSW) required.
  • Licensure:
    • Permanent LSW licensure required.
    • LCSW licensure preferred.
    • Temporary LSW license considered with full licensure expected within the first year.
  • Experience & Skills:
    • 0-3 years of relevant healthcare, mental health, or community-based experience required.
    • Knowledge of disease impact, human behavior, and legal mandates related to child and adult protective services.
    • Strong skills in crisis intervention and de-escalation techniques.
    • Ability to develop and maintain strong relationships with community agencies, law enforcement, schools, and healthcare providers.
    • Proficiency in Microsoft Excel and Word.
