Position Overview:
The Virtual Integrated Behavioral Health team provides innovative, evidence-based services through a virtual model, partnering with outpatient providers to support patients with mental health and substance use disorders. Responsibilities include screening, assessment, engagement, follow-up care, medication support, relapse prevention planning, and consultation with psychiatric providers. The role involves brief interventions, crisis management, and developing recovery plans for patients who have achieved their treatment goals.
This remote position requires effective communication and collaboration with healthcare teams, community agencies, law enforcement, and protective services, ensuring safe, timely, and comprehensive care.
Key Responsibilities:
- Collaborate with outpatient providers to support patients' mental and physical health.
- Screen and assess for mental health and substance use disorders; facilitate engagement and follow-up.
- Use brief, evidence-based behavioral interventions and consult with psychiatric specialists.
- Track treatment response and support psychotropic medication management.
- Develop relapse prevention plans for patients meeting treatment goals.
- Interpret and communicate complex patient/family needs; model caring practices.
- Provide mental health assessments upon physician referral.
- Serve as a resource and point of contact for cases involving child abuse/neglect, adult/elderly abuse, institutional abuse, and domestic violence.
- Liaise with protective teams, guardianship, foster care, and mental health placement agencies.
- Support patients during acute crises, utilizing de-escalation and crisis intervention techniques.
- Maintain strong relationships with community agencies, law enforcement, and healthcare providers.
Qualifications & Requirements:
- Education:
- Master's Degree in Social Work (MSW) required.
- Licensure:
- Permanent LSW licensure required.
- LCSW licensure preferred.
- Certifications:
- De-escalation certification required within 6 months.
- Experience & Skills:
- 3-5 years of relevant experience in mental health, Department of Child and Family Services, adult protective services, Veteran's Administration, or substance abuse preferred.
- Experience working with geographically and socio-economically diverse populations.
- Knowledge of disease impact, human behavior, crisis intervention, and legal mandates related to protective services.
- Strong crisis management and de-escalation skills.
- Proficiency in Microsoft Excel and Word.
