Position Overview:
The Social Worker plays a key role in ensuring patients and families receive the necessary resources, education, and emotional support during their healthcare journey. This role involves assessing psychosocial risk factors, providing therapeutic interventions, and acting as a liaison for cases involving abuse, neglect, guardianship, foster care, and mental health placements. The Social Worker effectively communicates complex patient needs to the healthcare team, models caring practices, and supports complex discharge planning.

Key Responsibilities:
  • Collaborate effectively with all members of the healthcare team to ensure safe and timely patient care.
  • Interpret and communicate complex patient/family needs; model caring practices to colleagues.
  • Assess patients' and families' psychosocial needs, prior functioning, support systems, and reactions to illness.
  • Counsel patients and families regarding emotional, social, and financial impacts of illness/disability.
  • Access and mobilize community and family resources to meet patient needs.
  • Conduct mental health assessments upon physician referral and provide therapeutic interventions.
  • Serve as the point person for cases of child abuse/neglect, adult/elderly abuse/neglect, institutional abuse, and domestic violence.
  • Liaise with protective services, guardianship, foster care, adoption, and mental health placement agencies.
  • Support complex discharge planning and crisis intervention as needed.
  • Maintain strong relationships with community resources, law enforcement, schools, and other agencies.

Qualifications & Requirements:
  • Education:
    • Master's Degree in Social Work (MSW) required.
  • Licensure:
    • Permanent LSW licensure required.
    • LCSW licensure preferred.
    • Temporary LSW license considered with full licensure expected within the first year of employment.
  • Experience & Knowledge:
    • 0-3 years of relevant healthcare, mental health, or community-based experience required.
    • Knowledge of disease impact, human behavior, and legal mandates related to protective services.
    • Skills in crisis intervention and de-escalation techniques.
    • Strong interpersonal and community relationship-building skills.
    • Proficiency in Microsoft Excel and Word.
  • Additional:
    • De-escalation certification preferred.
