Social Worker (MSW) Full Time, M-F, 8AM-4PM, Chilton Medical Center
Job Description
Responsible for counseling, assessing, and assisting individuals/families or groups who are affected by illnesses or injuries, including rape, physical abuse, cancer, AIDS, and Alzheimer's, that may impair their ability to function normally.
Principal Accountabilities:
Primary focus is the psychosocial approach with patients and their families who are receiving palliative and end-of-life care in the acute care, inpatient setting.
- Provides psychosocial assessments to determine patient needs, provides counseling and supportive services to patients/families, makes referrals to Hospice, community resources and members of the interdisciplinary team.
- Engages with the interdisciplinary team composed of physician, PA and NP practitioners, chaplain, clinical ethicist, and hospice nurse as well as other team members within the hospital community as well as those external yet key members such as the guardian and others.
- Actively contributes to the patient's overall plan of care, participates in daily team meetings, schedules, co-leads case conferences and engages with the patient's in-patient as well as out-patient care team.
- Maintains statistical information and analysis of data, develops outcome and measures for program evaluation, program development and facilitation of program.
- Identifies and maintains funding sources which supports the Family Counseling & Guidance Program.
- Other relevant duties as assigned
Qualifications
Required:
1. MSW from an accredited school of social work.
2. NJ LSW or LCSW required.
3. Previous medical hospital social work and grant experience strongly preferred. .
Preferred:
1. Maintains current knowledge of trends and advances in clinical practice and healthcare informatics, as well as new developments and innovations in hardware and software technology.
2. Demonstrated initiative, problem identification, resolution and analytical skills are essential, as well as excellent oral and written communication skills
